If you are a healthcare professional, we will collect your personal identifiers (name, physical address, telephone number, email address, Tax Identification Number, NPI, and signature). We use this personal information to assist you with your coverage and reimbursement for Digosure products and to communicate with you.
  • Contact us: When you contact us, we collect your personal identifiers (name and email address) and any other information you choose to include in your correspondence. We use this personal information to respond to your questions or inquiries, troubleshoot where necessary, and address any issues you may have.
